Ther is abnormally dilated veins seen without and with valsalva showing evidence of reflux on valsalva maneuver.

left testicular veins: Dilated 2.7 mm
Ther is abnormally dilated veins seen without and with valsalva
showing evidence of reflux at rest and on valsalva maneuver.
Conclusion: A case of Bilateral Varicocele grade III in left grade I in right bilateral epididymitis
Bilateral relatively small sized both testicles
(both testicles has normal vasculature on doppler study)
